Request for Correction of Information

Requests for correction of information should be addressed to:

A request for correction of information must include the following:

  • Specific reference to the information being challenged.
  • A statement specifying why the complainant believes the information fails to satisfy the standards in the Departmental or OMB guidelines.
  • Describe how the complainant is affected by the challenged information.  The complainant may include suggestions for correcting the challenged information; however, it is not mandatory.
  • The name and address of person filing the complaint.  This information is used at the complainant's request for the purpose of responding to the challenge initiated by the individual.

Appeals Process

If complainants are dissatisfied with the OS response to the request for correction, an appeal may be submitted for consideration 21 calendar days after receipt of the determination. 

The appeal request must contain the following:

  • Specific reference to the information being appealed, including the date of the original submission and date of the OS decision.
  • Explanation of why the complainant disagrees with the OS decision.  The complainant may include the recommendation for corrective action; however, it is not mandatory.
  • The name and address of person filing the complaint.  This information is used at the complainant's request for the purpose of responding to the challenge initiated by the individual.
  • A copy of the original request for the correction of information.
